Cervical encirclage (Os tightening)

Cervical cerclage, also known as cervical encirclage or os tightening, is a surgical procedure used to prevent preterm birth or miscarriage in women with a weakened or incompetent cervix. This condition can cause the cervix to open prematurely during pregnancy, leading to potential complications. Gynecologists perform this procedure to reinforce the cervix, thereby helping to maintain the pregnancy until term.

Purpose of Cervical Cerclage (Os Tightening)

  • To prevent premature dilation of the cervix in pregnant women who are at risk of preterm birth or miscarriage due to cervical incompetence.

Indications:

  • History of second-trimester miscarriages due to painless cervical dilation.
  • Ultrasound evidence of a short cervix (<25mm) during pregnancy.
  • Previous preterm birth associated with cervical incompetence.
  • Diagnosis of cervical insufficiency based on clinical history and findings.

Types of Cervical Cerclage

  • Prophylactic (Elective) Cerclage: Performed at 12-14 weeks of pregnancy in women with a known history of cervical insufficiency.
  • Therapeutic (Emergent) Cerclage: Performed when cervical insufficiency is diagnosed during pregnancy, often between 16-24 weeks.
  • Rescue (Emergency) Cerclage: Performed when there is significant cervical dilation or prolapse of membranes, typically beyond 24 weeks.
